The pandemic restricted in-person access to health professionals for many. In response, we expanded the list of health practitioners who could complete an Attending Physician’s Statement.

Form completion by physician or nurse practitioner only

With most Canadians now vaccinated, people can more readily access in-person health care. Effective November 1, 2021, we’re going back to our normal process. This means that only a physician or nurse practitioner can complete an Attending Physician’s Statement.

We will continue to accept the Confirmation of Illness form for short-term disability claims related to COVID-19. Plan members should use this form when COVID-19 symptoms or a COVID-19 diagnosis causes their absence.

We encourage you to let plan members know about this change. We’ve prepared this short plan member memo to help you do this. If we have an email address for plan members, we’ll also be sending the memo to them directly.
