What’s the Diabetes Care Program?

The program gives Sun Life Clients living with type-1 or type-2 diabetes online access to trained healthcare professionals – at no cost.

Care Programs are offered through the Online Pharmacy, by Pillway. When you enrol, you’ll also get access to the Online Pharmacy including online prescription refills, pharmacist support and free medication delivery. Medication coverage depends on your benefits plan.

Available to all Canadians, except those in Quebec.

Narrator: Here's how the program works.

You’ll meet with a pharmacist for a 30-minute virtual consultation. During this time, the pharmacist will complete an assessment to understand your personal health goals and how you are currently managing your diabetes.

Following the consultation, you’ll receive a personalized care plan with actions designed to help you achieve your goals.

If you’re using a tool to monitor your glucose levels in real-time, it may be able to seamlessly integrate into your plan; allowing your pharmacist to further personalize their recommendations for you as you progress through the program. A free trial kit for a continuous glucose monitor might be available. Ask your Pillway pharmacist during your consultation.

Your pharmacist will continuously update your care plan and schedule regular check-ins to provide coaching, proactive support, and education tailored to your needs. All of this is easily accessed through the Online Pharmacy, by Pillway.

This program is available to Sun Life plan members and their dependents at no cost and getting started is simple! Just sign in or complete your profile on the Online Pharmacy app or website, and book your 30-minute consultation.

Access comprehensive diabetes care – what’s included:

Personalized plan

  • 1-on-1 coaching from pharmacists with training in diabetes care.
  • Get a customized Care Plan built just for you - like your blueprint for success, designed to optimize your health.
  • Your Care Plan outlines your treatment and personal health goals, as well as personalized advice and recommendations on medication therapy, nutrition, monitoring and more.

Simple monitoring

  • A sensor-based or continuous glucose monitoring system can be used to integrate your glucose readings with the Care Program for highly personalized advice and recommendations.
  • During your initial consultation, a pharmacist will check your eligibility for a free trial kit of a sensor-based or real-time continuous glucose monitor (CGM), if you aren’t already using one.

Professional healthcare support

  • Meet your support team of licensed pharmacists from Pillway, with training in diabetes care.
  • Pharmacists are an important member of your care team. They’ll help you understand your assessments, medication regimen and treatment plan. Plus, they can share updates and recommendations with your Primary Care Provider, with your permission.
  • Your pharmacist support can extend beyond medication therapy. They can help direct you to other online programs and providers within your community, or they may direct you to other healthcare providers that can conduct further evaluations – such as optometrists or podiatrists.
  • Plus, you may have access to physicians and registered dieticians through LifestyleRx – an evidence-based, lifestyle medicine program that support individuals living with diabetes – depending on your province of residence.1

1. Sign in or register for Online Pharmacy by Pillway

  • Diabetes Care Program is offered through the Online Pharmacy by Pillway.
  • Registering is easy and only takes a few minutes.
  • You’ll need to provide your name, contact information and drug card.

2. Book your 30-minute consultation

  • Log-in and navigate to the “Pharmacist consultation” section and choose “Diabetes consultation.”
  • Complete the short consultation form.
  • Pick a date and time that works for you.

3. Get the ongoing support you need

  • Your personalized program lasts as long as you need support.
  • For ongoing support and advice, reach out to your pharmacist, anytime.

What’s the Online Pharmacy, by Pillway?

The Online Pharmacy is a pharmacy platform that offers an easy way to get your medications without having to go to the store. It also offers care programs, like the Diabetes Care Program, Respiratory Care Program and Can Quit Program, for ongoing support.

 Additionally, you can:

  • Easily fill and refill prescriptions.
  • Get connected care. Pharmacists can contact your Primary Care Provider should there be any changes to your health status or medication needs.
  • Your medications will be delivered for free – as soon as next day.
  • Contact the pharmacy for support with your medications.
  • Consult with pharmacists with training in chronic disease care.

Frequently asked questions

  • Your program support team may make recommendations. However, it will generally be your Primary Care Provider (e.g., family physician) that will adjust your medications, as necessary.
  • When enrolling in the program, the form requests your Primary Care Provider’s information. Your support team will keep them informed.
  • With your consent, your Primary Care Provider will be notified after every consultation.

Clients without a Primary Care Provider are still eligible for the Diabetes Care Program.

Looking for support in lowering your blood glucose levels, or putting type 2 diabetes into remission? Your dedicated support team will create your plan to help you establish sustainable habits with nutrition, physical activity, stress management and sleep. With your care plan and 1-on-1 coaching, it may be possible to lower your A1C levels, reduce the medications you are taking and put type 2 diabetes into remission.**

Narrator: Type 2 diabetes is a chronic condition that affects millions worldwide. But did you know it's possible to change its course?

The progression of type 2 diabetes often comes down to how our bodies use insulin less effectively over time, leading to higher blood sugar levels. This can lead to serious health complications. But there's hope.

In some cases, it is possible to put type 2 diabetes into remission; meaning that blood sugar levels return to below the diabetes range without the need for diabetes medications for at least three months.

Any action that leads to sustained weight loss of 15% or greater of your current body weight could potentially lead to remission of type 2 diabetes.

Remission may not be possible for some individuals with type 2 diabetes. Ask your healthcare professional for more information.

While remission isn’t a cure, it minimizes the risks of developing the complications of diabetes.

** Type 2 diabetes remission is defined as achieving specified glycated hemoglobin (A1C) thresholds without any antihyperglycemic medications for a minimum of 3 months: remission to prediabetes (A1C between 6.0% and 6.4%) and remission to normal glucose concentrations (A1C <6.0%).
