Submit or track a claim

Coverage obtained through your advisor

Contact your advisor directly. If you need your advisor's contact information, go to Find an advisor and enter your advisor's last name.

You can also call the Customer Care Centre at 1-877-SUN-LIFE (1-877-786-5433), Monday to Friday, 8am to 8pm ET.

Coverage obtained through your employer, online or by phone

For health or dental claims:

If you haven't yet signed up for direct deposit or online claims, please take a few minutes to register now. You will need your contract number and member ID number, which you will find on your coverage card, your claim statement or your new plan member welcome letter.

If you are already registered:

  1. Sign in to mysunlife.ca.
  2. Under my health and well-being, click my claims.
  3. If the header Submit a claim appears, you can submit a claim online.
  4. Choose the appropriate claim type (e.g., paramedical, vision) and follow the steps to submit.

If the Submit a claim header does not appear, you can’t submit a claim online. Return to the previous screen and select I want to submit a paper claim.

Register before you download:

Your pharmacist or dentist may submit some claims directly on your behalf. All you need to do is show them your coverage card.

To access your coverage card:

  1. Sign in to mysunlife.ca.
  2. Under my health and well-being, select Print drug card or Print travel card under the Take me to drop-down menu.
  3. Click the Print button located on the top right-hand corner of the web page.

By downloading our mobile app, you can also use your smartphone as your coverage card.

If you are making a claim for a service you can't submit online, you can print out a paper form that includes your personalized information and contract number.

To access your claim forms:

  1. Sign in to mysunlife.ca.
  2. Under my health and well-being, select Submit a claim from the Take me to… drop-down menu.
  3. Select Claim form from the list.
  4. Select the appropriate PDF claim form and fill out your claim information, or print and fill it out manually.
  5. Print and sign the form, attach all original receipts and mail it to the address on the form.

Remember to submit a signed claim form and the original receipts, or a copy of the claim statement from the insurance provider you first submitted your claim to if this is a coordination of benefits claim.

Generic group claim forms

If your organization does not offer personalized claim forms, you can get the most popular generic group claim forms.

To check the status of a claim:

  1. Sign in to mysunlife.ca.
  2. Click My claims.
  3. Under View a claim statement click Recent claims.
  4. If you have submitted a claim that has not yet been completed, click In progress to view its status.

Not registered online yet? Take a few minutes to register now. You can also call the Customer Care Centre at 1-800-361-6212, Monday to Friday, 8am to 8pm ET. Please have your coverage card ready.

To access your coverage card:

  1. Sign in to mysunlife.ca.
  2. Under my health and well-being, select Print drug card or Print travel card under the Take me to drop-down menu.
  3. Click the Print button located on the top right-hand corner of the web page.

For life insurance or accidental death and dismemberment (AD&D) claims:

For critical illness insurance claims:

Contact us at 1-800-669-7921 Monday to Friday, 8:30 am to 5pm ET.

For disability insurance claims:

Contact us at 1-800-361-6212 Monday to Friday, 8am to 8pm ET.

If you are with the Public Service Health Care Plan (PSHCP) or the Pensioners’ Dental Services Plan (PDSP) contact 1-888-757-7427 Monday to Friday, 6:30 am to 8pm ET.
