New to Choices: What to do after enrolling in your plan

Congratulations on making the important decision to enroll in a Sun Life Financial Choices Plan. To help you get off to a great start and make the most of your plan, we’ve created this checklist for what to do next.

1. Complete and submit the Beneficiary Designation Form (savings and life insurance plans only)

When you move your money from a workplace savings plan to a Choices plan, your original beneficiary designations are not automatically transferred, unless the nominations under the group savings plan were irrevocable.* You must choose a new beneficiary by completing the Beneficiary Designation Form that will be included in your Choices Welcome Package. If you do not designate a beneficiary, the proceeds of your Choices Plan will be paid to your estate in the event of your death. Alternatively, you can find the Beneficiary Designation Form by signing in to sunlife.ca/choices.

After logging into the website, click on Profile (at the top of home page) > Personal Info > Beneficiary Info. You can also call 1-877-736-4741 to request a copy of the form, or to set up your access to mysunlife.ca if you have not yet done so.

*Your Financial Services Consultant (FSC) will have confirmed whether your group beneficiary designation(s) was/were irrevocable during your enrollment call. If you wish to confirm this information after enrollment, you may call the Sun Life Financial Customer Care Centre at 1-877-736-4741.

2. Complete and submit an Authorized Chequing Plan (ACP) form (savings plans only)

If you decide you’d like to set up automatic contributions from your bank account after you’re enrolled in the plan, you can complete and submit an Authorized Chequing Plan form. Please call the Sun Life Financial Customer Care Centre at 1-877-736-4741.

3. Complete and submit a Pre-Authorized Debit (PAD) form (life/health benefits plan only)

If you are enrolled in a Choices life/health benefits plan and prefer not to make payments via credit card, or have declined/been declined Equifax authorization, you may set up your payments by preauthorized withdrawals from your bank account. If you did not provide payment information at enrollment, a Pre-Authorized Debit form will be mailed to you. To request a copy of this form, you may also call 1-877-736-4741.

4. Review your personal contact preferences

We want all of our communications to be relevant and valuable to you. As a Choices plan member, Sun Life Financial may contact you from time to time to provide you with information, offers and options to help you get the full value of your plan(s)/policy(ies). We may also send you information about products and services we believe meet your changing requirements, or surveys to help us better understand your needs. Please tell us how you would like us to communicate with you, and what types of information you prefer to receive, by visiting the contact preferences page on sunlife.ca/choices. After logging into the website, click on Profile (at the top of home page) > Personal Info > Privacy and Contact Preferences > link to Contact preferences page.

5. Stay connected to your plan

Whether you’ve transferred assets into a Choices savings plan or protected your finances and family with a life/health benefits plan, your investment and/or insurance coverage needs may change over time. It’s a good idea to review any investment or benefits plan regularly, to ensure it still meets your needs.

Log into sunlife.ca/choices anytime, anywhere to see what’s new with your Choices savings plan; review your investment mix and performance; make contributions or update your future investment instructions; calculate your retirement income needs and much more. If you have money invested in other plans outside of Sun Life Financial, and you’d prefer to have all your investments under one umbrella plan, you may transfer that money into your Group Choices Plan at any time. You can also call 1-877-805-9303 to speak with one of our Financial Services Consultants, who can help you ensure you’re getting the most out of your Choices plans and make any changes needed. We’re available to take your call, Monday through Friday, 8 a.m. to 6 p.m. ET.

We can also help you with your health benefits, even if Sun Life wasn’t the provider of your workplace plan, and it’s still within 60 days of the date your workplace coverage ended.
