Definition of terms

Declined: Insurance coverage was applied for and the company did not offer coverage.

Elevated cholesterol: an excess of fats or lipids in the blood. It is also called hyperlipidemia or hyperlipemia.

Elevated blood sugar: an increase of sugar (glucose) in the blood. The Canadian Diabetes Association and World Health Organization define diabetes as a fasting blood sugar of greater than 7.0 mmol/L.

Medical equipment: any medical equipment used to enable mobility and functionality (e.g., wheelchair, hospital bed, traction apparatus, continuous positive air pressure [CPAP] machines, etc.)

Medical testing device: any medical device used to enable assessment of symptoms (e.g., lancets, syringes used in monitoring blood sugar, blood pressure monitor, holter monitor, etc.).

Medication: refers to any drug or substance, whether prescription or non-prescription, taken to reduce symptoms or cure an illness or medical condition.

Modified: refers to an insurance plan that excludes from coverage specific conditions/situations included in the plan applied for or that requires a higher premium be paid.

Pre-existing condition or treatment: any injury, illness, disease or sickness of which there are symptoms, or for which medical treatment, care, advice or diagnosis was recommended or received before the date the application was signed.

Rated: refers to an insurance plan that requires a higher premium be paid for the extra risk associated with health-related factors.

Substance abuse: refers to the regular or frequent use of an illegal or harmful substance, usually alcohol or a drug, or the use of prescription or non-prescription drugs for purposes or in quantities other than directed.
