Sometimes the pharmacy can’t automatically verify your eligibility. When this happens, you can submit a form for manual review. 

Available eligibility verification forms:

Submit your completed forms by signing in to our mobile app

  1. Sign in to my Sun Life on mobile. 
  2. Go to Health, then to ‘Health actions’.  
  3. Then, select ‘Submit documents’.  

You can also mail or fax these forms to us. You’ll find the address information on the form.
