Forms related to coverage obtained through your employer

Avoid the hassle of paper claim forms by signing up for direct deposit and online claims submissions, if available. Your claims are processed instantly and your money is usually available within 48 hours. Sign in to mysunlife.ca. Not registered online yet? Take a few minutes to register now.

Note: For security purposes, we may send you a temporary password by mail, typically in 3 to 5 business days. Once you’ve registered and received your password, you can sign in to mysunlife.ca to find forms, and submit and track your claims.

If you are making a claim for a service you can’t submit online, you can print out a paper form that includes your personalized information and contract number.

To access your claim forms:

  1. Sign in to mysunlife.ca.
  2. Under my health and well-being, select Submit a claim from the Take me to… drop-down menu.
  3. Select Claim form from the list.
  4. Select the appropriate PDF claim form and fill out your claim information online, or print and fill it out manually.
  5. Print and sign the form, attach all original receipts and mail it to the address on the form.

If your organization does not offer personalized claims forms, you can get the most popular generic group claims forms*.

*Most employers and other organizations offer the option to submit your claims online, or they provide personalized claims forms that help speed up the processing of your claim. If your group plan uses personalized forms but you submit a claim on a generic form, the same terms and conditions apply as set out in your plan sponsor’s personalized form.
