Simplified plan member consent for Short-Term and Long-Term Disability
At Sun Life, we’re committed to protecting the confidentiality of our plan members’ information. This commitment means being completely transparent about how we collect and how we use their personal data. We've recently made updates to our plan member consent included in the Plan Member’s Statement for Short-Term Disability (STD) and Long-Term Disability (LTD). We’ve also refreshed our STD and LTD guides included in the standard plan member packages.
What’s changing
The revised consent now uses clearer language and a more user-friendly format. These changes aim to:
- Help plan members better understand how Sun Life collects, uses and discloses their personal information related to their disability claims.
- Provide clarity on purpose, sources of information, and duration of consent.
Please note that we’re not making any changes to how we collect, store, or use your plan member’s information.
The updated guides now reflect our revised medical statement, ensuring consistent language through the plan member package (which includes the guide, plan member statement, and the Initial Disability Insurance Medical Statement).
Next steps
No immediate action is required from you or your plan members. You can access the updated plan member’s disability claim forms here. The new forms are also available on the plan member website.
If you are a Client with a customized plan member disability claim form, this change does not affect you. The change only applies to our standard disability forms mentioned above.
