  • If you require a paper form, please visit the find a form page to find the claim forms you need.

  • If you are with the Public Service Health Care Plan (PSHCP) or the Pensioners’ Dental Services Plan (PDSP) contact 1-888-757-7427 Monday to Friday, 6:30 a.m. to 8 p.m. ET.

For Life or Accidental Death and Dismemberment (AD&D) claims:
