Member means the person who belongs to the group benefits plan.

Dependent(s) means your spouse and children who are eligible under the terms of your group benefits plan.

Spouse means your spouse by marriage or under any other formal union recognized by law, or a person of the opposite sex or of the same sex who is publicly represented as your spouse according to the terms of your group benefits plan. Only one person at a time can be covered as your spouse.

Effective date means the date when insurance coverage begins according to all the terms of your group benefits plan.

Doctor means a physician or surgeon who is licensed to practice medicine where that practice is located.
