Pre-screening checklist

Sun Life Financial has created this document to help advisors determine which risks are uninsurable for long term care insurance.

Stability periods are a very unique and important component of long term care insurance (LTCI) underwriting. Generally speaking, a stability period refers to the length of time a condition has been well-controlled:

  • without any change in the condition or symptoms and
  • without any change in treatment.

Refer to the 'Long term care insurance field underwriting guide' for more information and to determine if the stability period has been met for the applicant's particular condition. If the client has not met the required stability period please defer completing the application until the stability period has been met.

Uninsurable conditions: If the client has any of the following conditions, they are uninsurable for long term care insurance coverage with Sun Life Financial. Please do not submit an application.

  • AIDS or HIV positive
  • Alzheimer's disease
  • Amyotrophic lateral sclerosis (ALS or Lou Gehrig's disease)
  • Chronic congestive heart failure
  • Cirrhosis of the liver
  • Cystic fibrosis
  • Degenerative disc disease (severe)
  • Dementia/senility
  • Huntington's disease
  • Incontinence
  • Kidney/renal failure
  • Marfan's syndrome
  • Memory loss
  • Multiple myeloma
  • Multiple sclerosis
  • Muscular dystrophy
  • Osteoarthritis (severe)
  • Osteoporosis (severe)
  • Paraplegia, hemiplegia, quadriplegia
  • Parkinson's disease
  • Post polio syndrome
  • Schizophrenia
  • Sickle cell anemia
  • Spinal stenosis (moderate or severe)
  • Systemic lupus erythematosus
  • Multiple transient ischemic attack (TIA) and/or stroke or cerebrovascular accident (CVA)

Some of the more common uninsurable medications: If the client is taking any of these medications they are uninsurable for long term care insurance coverage with Sun Life Financial.

  • Aricept
  • Haldol
  • Insulin - 40 units or more per day
  • Methotrexate - over 15 mgs/week
  • Prednisone - over 6 mgs/day
  • Rebretron
  • Respiridone
  • Risperdal
  • Seroquel
  • Sinemet
  • Ziprasidone
  • Zyprexa

Uninsurable risk factors: If the client has any of the following risk factors, they are uninsurable for long term care insurance coverage with Sun Life Financial at this time.

  • Pending medical tests, investigations, referrals or surgery
  • Age 65 and older and has not had a complete physical within the last three years
  • Currently using an assistive device (e.g. wheelchair, walker, motorized scooter, multi-pronged cane)
  • Currently using respirator/oxygen or dialysis
  • Currently not working and receiving disability benefits
  • Requires assistance for bathing, dressing, toileting, transferring, feeding
  • Unable to perform more than one of the following: using the telephone, managing finances, taking transportation, shopping, laundry, housework, preparing meals/cooking, or taking medications
  • Requires care services such as adult day care, home health care, assisted living or care provided in a nursing home
  • Lives outside of Canada for more than six months per year
  • Has used tobacco products in the last 12 months and has a history of:
  • aneurysm
  • arteritis
  • carotid artery disease
  • coronary artery disease (CAD)
  • cardiomyopathy
  • congestive heart failure (CHF)
  • diabetes
  • lung, throat or mouth cancer
  • lung disease
  • neuropathy
  • peripheral vascular disease (PVD)
  • stroke

Build: If the client's build is greater than the maximum weight on the following chart, they are uninsurable for long term care insurance coverage with Sun Life Financial.

Note: If the client has lost weight within the last year, add half the amount of weight lost to their current weight.
