Juvenile smoking status change

Only use the Juvenile declaration of smoking status
if the life insured was a child when the policy was issued.

Policies insuring children automatically have a smoker classification. A smoker rate is charged if the policy owner doesn't send Sun Life a completed declaration at the times set out below; or the completed declaration states the insured is a smoker. If the form states the life insured is a non-smoker, we will remove the smoker classification.
