The "Term Insurance+ program" (the “Program”) begins on May 16, 2022, at 8 a.m. ET and ends on August 31, 2022, at 11:59 p.m. ET (the “Program Period”). Sun Life Assurance Company of Canada (the “Program Sponsor”) sponsors the Program.
The Program is open to legal residents of Alberta, Canada. Residents must be 18 years of age or older. To be eligible for the Program, you must be a Sun Life life insurance Client who:
(the “Participant”, collectively the “Participants”)
Employees, representatives, or agents of the Program Sponsor or its affiliates cannot participate in the Program. Immediate family or those living in the same household as those employees, representatives or agents cannot participate in the Program. The expression “immediate family” means spouse, mother, father, sister, brother, son or daughter.
In the event that a person does not meet the eligibility requirements as set forth herein, Program Sponsor has the right, in its sole discretion, to reject or disqualify such person from participating in this Program. In the case of disqualification, the disqualified person will not be entitled to any benefit of the Program due to such disqualification.
In order to participate in this Program, a Participant must complete the following steps:
Step 1: Registration (the “Registration”)
a) Download the Optimity app (the “App”) on your mobile device before January 1, 2023,
b) Create an Optimity account using the Optimity invite code (the “Invite Code”) that you received from Sun Life to your email address you used in your life insurance application, and
c) Complete the following activities as part of the registration process.
i. Health Risk Assessment, and
ii. Set your health goals
Step 2: Earn up to $100 in gift cards
a) Participants who complete the Registration process (the “Eligible Participants”) can start earning gems by participating in activities including and without limitation to:
i. Steps
ii. Daily goals
iii. Learn and earn quizzes
iv. Activities
v. Mini-courses
vi. Challenges
vii. Logging workout
viii. Logging nutrition
ix. Tracking water
b) Eligible Participants can use gems to redeem gift cards (the “Gift Cards”) from the Rewards section of the App. The Gift Cards redemption is limited to $25 per person per month, and $100 per person per calendar year.
Step 3: Unlock a wellness solution
a) Eligible Participants can unlock one of the following wellness solutions (the “Wellness Solution”, collectively the “Wellness Solutions”):
i. Option 1 – Standard
ii. Option 2 – Choose Your Own
iii. Option 3 – Bring Your Own
b) Option 1: Eligible Participants who selected the Standard option will receive a Fitbit Inspire 2 (the “Standard Model”) at no extra cost. In order to receive the Standard Model, they must:
i. Opt in to “Option 1”,
ii. Provide their preferred address for shipping, and
iii. Complete the order within 20 days from the Registration.
c) Option 2: Eligible Participants who selected the Choose Your Own option will be able to pick a wearable device at a discounted price. In order to receive a wearable device other than the Standard Model, they must:
i. Opt in to “Option 2”,
ii. Choose a device from the catalog provided by Optimity,
iii. Enter their billing address and pay the difference in price including taxes (after the $100 discount) using one of the accepted methods by Optimity,
iv. Provide their preferred address for shipping, and
v. Complete the order within 20 days from the Registration.
d) Option 3: Eligible Participants who selected the Bring Your Own option will receive a $100 e-gift card once they sync their own wearable device with Optimity. In order to receive the $100 e-gift card, they must:
i. Opt in to “Option 3”,
ii. Sync their own wearable device that’s compatible with Optimity,
iii. Choose a $100 e-gift card from the catalog provided by Optimity,
iv. Enter their e-mail address, and
v. Complete the order within 20 days from the Registration.
e) Eligible Participants cannot alter their option once they have completed the order of the Wellness Solution. No substitutions are to be made save and except at the Program Sponsor’s sole and absolute discretion. The Wellness Solutions are non-transferable and non-assignable and can only be redeemed by Eligible Participants.
f) There is a limit of one (1) Wellness Solution per Eligible Participant.
g) It is the responsibility of Eligible Participants to ensure that contact information, including full name, email address, home address submitted is accurate and up to date. The Program Sponsor and/or its agencies or affiliates shall have no responsibility for any consequences resulting from Eligible Participants providing out of date or incorrect information.
In case an Eligible Participant cancels their policy or their policy lapses within the first year as a failure to pay monthly premiums, they will be considered to withdraw from the Program and the Program Sponsor has the right to either ask them to return the “Standard Model” or $100 provided as part of the Wellness Solutions.
This Program is subject to all applicable federal, provincial and municipal laws. The decisions of the Program Sponsor with respect to all aspects of this Program are final and binding on all Participants without right of appeal, including, without limitation, any decisions regarding the eligibility/disqualification of participation.
The Program Sponsor reserves the right, in its sole and absolute discretion to withdraw, amend or suspend this Program (or to amend these Terms and Conditions) in any way without prior notice.
By entering this Program, each Participant expressly consents to the Program Sponsor and Optimity, its agents and/or representatives, storing, sharing and using the personal information submitted with their participation only for the purpose of administering the Program and in accordance with the Program Sponsor’s privacy statement, unless the Participant otherwise agrees.
The Program Sponsor reserves the right, in its sole and absolute discretion, and without prior notice, to adjust any of the dates and/or timeframes stipulated in these Terms and Conditions. The extent of adjustments can be based on:
a) Purposes of verifying compliance by any Participant or participation with these Terms and Conditions, or because of any technical or other problems.
b) In light of any other circumstances which, in the opinion of the Program Sponsor, in its sole and absolute discretion, affect the proper administration of the Program as contemplated in these Terms and Conditions.
c) For any other reason.
In the event of any discrepancy or inconsistency between these Terms and Conditions and disclosures or other statements contained in any Program-related materials, these Terms and Conditions shall prevail, govern and control to the fullest extent permitted by applicable law.
All intellectual property, including but not limited to trade-marks, copyrights, patents, trade names, logos, designs, promotional materials, web pages, source code, drawings, illustrations, logos, slogans and representations are owned by the Program Sponsor. All rights are reserved. Unauthorized copying or use of any copyrighted material or intellectual property without the express written consent of its owner is strictly prohibited.
Fitbit, the Fitbit logo, and Fitbit Inspire 2 are registered trademarks of Fitbit LLC and/or its affiliates in the United States and other countries and regions.
The Program Sponsor and/or its agencies or affiliates will not be responsible for invalid, expired, unchecked information including full name, email address, and delivery address.
The Program Sponsor and/or its agencies or affiliates will not be responsible for any unavailability of or interruptions to any service or equipment used in connection with the Program, including and without limitation to:
a) Interruptions to any network, server, Internet, website, telephone, satellite, computer or other connections
b) Failures of any telephone satellite, hardware, software or other equipment.
c) Garbled, misdirected or jumbled transmissions or traffic congestion.
d) Other errors of any kind, whether human, technical, mechanical or electronic.
e) The incorrect or inaccurate capture of information or the failure to capture any such information.
For the Standard Model (Option 1) and Bring Your Own (Option 3) options, shipping, handling fees and GST (if applicable) will be paid by the Program Sponsor.
For the Choose Your Own option (Option 2), shipping and handling fees will be paid by the Program Sponsor. All applicable government taxes will be applied at the check-out and Eligible Participants will need to pay in full.
For residents of all other provinces and territories outside of Quebec, any litigation must be commenced in the City of Toronto, Ontario.
The Program is subject to all applicable federal, provincial and municipal laws.
By participating in the Program, Participants grant the Program Sponsor the right to collect, use, or disclose the personally identifiable information of Participants (collectively, the “Personal Information”) for the purpose of administering the Program, including but not limited to selecting Participants and fulfilling the Wellness Solution. The Program Sponsor will treat each Participant’s Personal Information in a manner consistent with the Program Sponsor’s privacy policy, accessible at https://www.sunlife.com/sl/pslf-canada/en/. Personal information collected during the Program fulfillment process is temporarily stored by a third-party vendor and may be in a jurisdiction different than the one the Participant is in and their Personal Information may be subject to the laws of the local jurisdiction.
