My Life Choice

Maintain up to $1,000,000 of individual Term Life Insurance and up to $250,000 of Accidental Death coverage for you and your spouse. Apply within 60 days of leaving your workplace plan. You and/or your spouse must be between the ages of 18 and 65.

My Health Choice

Get Extended Healthcare coverage – Standard or Enhanced – which includes dental coverage. Apply within 60 days of leaving your workplace2 plan. You and/or your spouse must be between the ages of 18 and 74. If you are outside of 60 days and in good health, you may qualify for a plan with more robust coverage and higher maximums.

Choices Critical Illness Insurance

Maintain up to $100,000 of coverage each for you and your spouse and $20,000 for each eligible child. Apply within 60 days of leaving your workplace2 plan. You must be between the ages of 18 and 69.

Provides insurance to cover the cost of certain in-home healthcare and home conversion products you may need to stay in your own home as you get older. You must be age 50+. Qualify within 60 days of leaving your workplace plan with no medical questions asked.
