Terminate a member - search

Enter:
  		• the last name or the member ID of the member you wish to remove from your group plan,
  		• the effective date of the termination and
  		• the termination reason by selecting from the list and
  		• the phone number of the member (home or cell).
    The wildcard (*) search is enabled. For example, to select all members with the last name of Smith, type "Smi*" in the last name box.
