Depending on your relationship with us, we offer several easy and convenient ways to find forms.

Forms related to coverage obtained through your employer:

  • Sign in to the Plan Member Services website. Don’t have an account? Register now.
  • Contact your plan administrator or the Customer Care Centre at 1-866-896-6976 between 8am and 8pm ET.

Have you left your employer?

  • If you've left your employer and need to obtain a form to convert your coverage, please call the Customer Care Centre at 1-877-893-9893 between 8am and 8pm ET.

Forms related to coverage obtained through your advisor:

  • Contact your advisor directly.
  • Call the Customer Care Centre at 1-877-SUN-LIFE (1-877-786-5433) between 8am and 8pm ET. Select your language preference followed by option 2.

Forms related to coverage obtained online:

  • Sign in to the Plan Member Services website. Don’t have an account? Register now.
  • Contact the Customer Care Centre at 1-800-669-7921 between 8:30am and 4:30pm ET.
