Health Coverage Choice
If you're a member of a company that provides group supplementary health, prescription drug and dental benefits and you leave that company, you can apply for Health Coverage Choice.
Health Coverage Choice provides affordable coverage for unexpected health expenses as well as many ongoing health-related needs not covered by provincial plans. If you don't have provincial health care coverage, we cannot provide you with Health Coverage Choice insurance.
Quebec residents must have the health and drug coverage through the Régie de l'Assurance Maladie du Québec (RAMQ) or through group benefits. If you have drug coverage with your group benefits instead of RAMQ, we need to know the policy and certificate number to direct claims to the group plan first. Health Coverage Choice is an individual plan and does not take the place of a group plan; you cannot opt out of RAMQ because you have HCC.
You have 60 days from the day your group coverage ends to purchase Health Coverage Choice. You may include other family members on your plan if they were insured through your group benefits plan. You and all persons included in your application must be age 74 or under on the date you apply. There are no medical requirements.
You can choose from 3 plans designed for optimum coverage.
These plans complement the coverage provided by your Canadian provincial plans. You must have provincial health coverage in order to qualify for Health Coverage Choice. You must apply to your province for reimbursement first. The unpaid portion can then be submitted for coverage under your Health Coverage Choice plan.
Health Coverage Choice is available for applicants up to age 74 and is renewable for customers age 75 and over. This policy is renewable yearly.
You are eligible for this insurance if your group supplementary health, prescription drug and dental benefits (if you choose the dental option) were in effect within 60 days of the date of your application.
